Incoming Freshmen Transfer Students

Freshmen Registration Days

INCOMING FRESHMEN and their parent/guardian are required to attend one of the following registration days:
  • July 17, 2012 (noon - 8 p.m.)
  • July 18, 2012 (noon - 8 p.m.)
  • July 19, 2012 (8 a.m. - 3 p.m.)

INCOMING FRESHMEN are required to bring the following documents:
  • Copy of the student's birth certificate
  • Freshman physical
  • Proof of residence
  • If necessary, proof of court appointed guardianship

(Physicals are required for all ninth graders by the state of Illinois. A copy of an appointment card that verifies an appointment has been scheduled for the physical exam is required. Students who cannot provide proof of an appointment will not be allowed to register.)
 

How to Transfer to MHS

TRANSFER STUDENTS are required to bring the following documents:
  • Copy of the student’s birth certificate 
  • If necessary, proof of court appointed guardianship
  • Physical examination and immunization report
  • Unofficial transcript from previous school
  • “State Transfer” form
  • “Student in Good Standing” form
  • Proof of Residence


Failure to bring all the required documents will result in the student not being allowed to register.

HOMEOWNERS MUST PROVIDE RENTERS MUST PROVIDE
  • Sales contract, mortgage papers, OR most recent real estate tax bill
Plus one more of the following documents:
  • current homeowner's insurance policy
  • current vehicle registration
  • current vehicle insurance policy
  • current driver's license
  • current water bill
  • current electric bill
  • current gas bill
  • current voter's registration card
  • Lease
Plus one more of the following documents:
  • current renter's insurance policy
  • current vehicle registration
  • current vehicle insurance policy
  • current driver's license
  • current water bill
  • current electric bill
  • current gas bill
  • current voter's registration card
