Request for Transcript Instructions
Please

Transcript Request Form.” (pdf file).
In order for your request to be processed you
MUST follow the instructions below. All incomplete requests will be ignored.
Your request must contain your original signature and can be mailed to the school, faxed or scanned/emailed to:

Alumni can disregard the sections referring to student id, counselor name and counselor initials. Please use your maiden name when requesting transcripts if your last name has changed since exiting high school. Please provide a contact phone number if staff have issues processing Alumni requests.
Students requesting a transcript who are younger than 18 years of age must have a parent/guardian signature on the request form. Current students should meet with their guidance counselor to discuss their post secondary plans prior to submitting a transcript request form.
Students that are 18 years old or older must complete their own request with their own signature. The law does not allow for parents to complete a request on behalf of students that are 18 years old or older.
For each transcript you are requesting a separate form needs to be completed. For example, if you are applying to 4 colleges you will need 4 separate completed request forms.
Unweighted = GPA does not include Honors or Advanced Placement classes.
Weighted = GPA does include Honors or Advanced Placement classes.
Please be aware that
our transcripts do not include ACT scores. Scores can be obtained from
www.act.org for a fee.
Allow (5) business days for your request to be completed.
Graduating Students
All graduating students
MUST complete the
Final Transcript Request Form (pdf). This form must be completed and include the college or agency to receive the FINAL transcript (which will include grades and class rank). Estimated time for final transcript mailing is June 15 of the current year.
Students will not be able to register at the college/university of their choice in the fall without this final transcript form being completed.
